Failure Analysis

Note: Failure Analysis is only available if the Criticality Configuration module is enabled. Check your On Key license to determine whether you have access to this functionality.

Failure analysis enables you to analyse specific asset or asset type component failures. Effective failure analysis will help to prevent common component failure modes from occurring by identifying the following:

Root cause of the component failure
Preventative tasks required to prevent future failures
Repair types required in the event of the failure

In order to capture comprehensive failure analysis information for a component, you need to configure the following information:

Failure mode — Describes how the component failed
Root cause — Identifies the underlying cause of the failure
Repair type — Describes the type of repairs required
Tasks — Identifies the tasks required to prevent the failure from re-occurring

The diagram below shows the relationship between the various failure analysis components.

For example, say the bearing component of a pump asset has seized as a result of poor lubrication. The failure analysis for this component failure mode could be described as follows:

Failure mode — 'Seized - Mechanical failure'
Root cause — 'Insufficient lubrication'
Repair type — 'Replace bearing and lubricate'
Tasks — 'Inspect bearing lubrication'
