The name of the person who initiated the Request, defaults to the user’s User Name. Select a code from the lookup list. [OK5R5] This person is identified as the Requester in On Key Extensions. |
Requested On |
The date and time on which the Requestor requested service. |

The Requestor’s contact details. |

The Work Order that has been raised to repair the Asset that failed, and solves the problem, if applicable. The Work Order's Status is displayed. Select the applicable Work Order from the lookup list if it already exists. |
